Clinical Manager
Patient Care Solutions LLC is currently seeking an experienced Clinical Manager. You will play a crucial role in delivering outstanding patient care while leading a dedicated team. This position requires strong organizational skills and a comprehensive understanding of in-home care practices. You will be responsible for overseeing clinical operations, ensuring compliance with federal and state standards, and coordinating patient care with an interdisciplinary team. Your leadership will make a significant impact on the quality of care provided to patients in their homes.
As a Massachusetts-based homecare agency, our primary goal is to ensure our patients can live healthy and fulfilling lives in their own homes. Our agency is nationally accredited by Community Health Accreditation Program (CHAP) and Certified by Medicare to provide homecare services throughout the state.
Responsibilities:
- Responsible for OASIS collection (data entry, editing, and transmission).
- Maintain agency compliance with above at or greater than 90% compliance. Self assesses compliance on a monthly basis and forwards a lesser compliance rating to the PI Committee for action plan formulation.
- Set up new client clinical records for home office and other provider branches as requested.
- Break down discharged clients records and give them to the Administrator.
- Oversee physician orders and mail orders to the appropriate physicians.
- File documentation on client charts in chronological order in appropriate locations, daily.
- Maintain files in locked cabinets to assure confidentiality of client information.
- Assist office and field staff in locating charts and documentation.
- Takes responsibility for ensuring that the pre-bill process occurs prior to submitting the final Medicare bill.
- Develops training process and timeline for implementation of medical systems. Accepts primary responsibility for training professional field staff in medical system use, uploading, and correct coding.
- Works in conjunction with PI Committee, recognizing the importance of oversight, improvement of processes and overall growth and success of the Agency.
- Works to develop a positive rapport with all staff members recognizing their importance to effective team building.
- Assists with the orientation of new professional employees relative to HIPAA, OASIS compliance and utilization of the EMR (electronic medical record).
- Perform other related duties as assigned by the administrator.
- Coordination and supervision of clinical documents into medical records as it pertains to document Manager and clinical explorer in your system.
- Responsible for effective coding of client diagnosis to maximize reimbursement in a clinically and ethically appropriate manner.
- Supervises data entry clerks inputting timesheets. Provides assistance, direction and review of weekly payroll entries. Has primary oversight of the FAS entry process, ensuring timely completion of the FAS process and preprocessing review prior to information transferring to the payroll function.
- Responsible for meeting the weekly payroll transmission deadline.
- Manages, evaluates and troubleshoots clinical record processes for effectiveness, efficiency and accuracy. Notifies appropriate personnel in writing regarding late paperwork, missed visit documentation etc.
- Demonstrates leadership and responsibility to the management team functioning.
Qualifications:
- Bachelor's in Nursing (BSN) or equivalent.
- Registered Nurse (RN) in Massachusetts.
- 3+ years of experience in a home health care setting.
- 2 years experience in the management of OASIS, including ICD-10 coding.
- Must possess excellent organizational skills and knowledge of numeric and alphabetical filing systems.
- Must exhibit maturity and ability to deal with people effectively.
