Riverside Regional Medical Center in Newport News, Virginia is currently seeking a labor pool (as needed) Clinical Pharmacist.

Requirements:
- Graduate of school of pharmacy required. If a foreign graduate of pharmacy, has completed all requirements for equivalency as outlined by the National Association of Board of Pharmacy (NABP).
- Licensed or eligible by the State of Virginia as a pharmacist required.
- Previous hospital pharmacy experience strongly preferred.
- Prefer candidate that has completed a PGY1 residency.
Overview
Works under the supervision of the Director/Manager of Pharmacy. May be designated by the Director/Manager of Pharmacy, or designee, as being in charge. Supervises and performs compounding (both sterile and non-sterile), packaging and dispensing of pharmaceuticals according to prescription, physician order or floor stock request in compliance with state and federal law and regulation. Review, validation and patient-specific assessment of medication orders. Monitors patient profiles to assure appropriate dosage and therapy. Provides current drug information. Demonstrates the knowledge and skills necessary to provide care appropriate to the age group of the patients served including participation in pharmacy clinical programs. Possesses the skills and ability to assess data reflective of patient status and to interpret this information and identify the patients' needs in relation to age-specific requirements, and to provide care as described in accordance with state and federal law and regulation, and facility policy and procedures. Supervises the work of pharmacy technicians, interns, and students. Performs other duties as assigned.
What you will do
- Supervises and performs compounding (both sterile and non-sterile), packaging, and dispensing of pharmaceuticals according to prescription, physician order, or floor stock request in compliance with state and federal law and regulation.
- Reviews, validates, and assesses patient-specific medication orders. Monitors patient profiles to assure appropriate dosage and therapy.
- Responsible for all core clinical pharmacy programs and commitment to evidence-based patient care, including but not limited to IV to PO, renal/hepatic dosing, therapeutic interchange, antimicrobial stewardship, anticoagulation management, duplicate therapy, drug-drug interactions, non-formulary management, and weight-based dosing.
- Handles all aspects of pharmacy operations to include order review and validation, sterile and non-sterile compounding, controlled substance/diversion management, pharmacy automation, and all state/federal law and regulation.
- Establishes an environment of safety and assures safe practices for the patient as evidenced by recognizing and implementing safe medication practices, and following policies and procedures that comply with regulatory agency regulations.
