Weekend Clinical Resource Nurse - Full time
Reporting to the Hospice House Manager, the Clinical Resource Nurse is responsible for providing support for home care and hospice patients through telephone triage. This position is instrumental in assessing and prioritizing patient care needs. This role is essential in providing interdepartmental collaboration and support. The Clinical Resource Nurse works collaboratively with home care and hospice leadership and clinical teams.
Essential Functions
- Clinically triages phone calls from patients/families/caregiver by providing skilled and palliative care assessment, problem solving and instruction along with assisting to coordinate home visits as indicated for patients who call the agency seeking guidance.
- Clinically coordinates scheduling for complex IV, Medication Reconciliation and Wounds Care referrals
- Provides guidance including pain management and symptom control; conducts assessments and evaluations and provides education and support care to patients and caregivers in collaboration with the nurse case manager.
- Coordination of patient care with the physician office and enters orders as indicated. Communicates outcome with clinical team.
- Receives and contacts by phone/fax/email any patient physician (designee) as indicated by the change in the patient’s condition, treatment and/or status.
- Completes comprehensive documentation about the calls received/initiated in the patient’s medical record using the SBAR guide.
- Assists daily with clinical triage of community and facility nursing and therapy visits and admissions.
- Supports scheduling with determining priority of assigning open visits.
- Reviews orders received by intake that impact the patient’s plan of care and makes appropriate changes
- Responsible for following safe work practices and company rules, and for preventing accidents and injuries.
- Manages and addresses all Hospice House and Hospice Community referrals and admissions and coordinates with intake team and providers. Accepts eligible patients at each level of care and triage according to need and bed availability with weekend providers.
- Sends weekend report to Hospice House Manager on team member issues, patient care and unusual events.
- Acts as a primary resource for Hospice related clinical care triage and questions.
Requirements
Minimum Qualifications and Requirements:
- Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
- Registered Nurse with one-two years of home health or hospice experience.
- One-two years of telephone triage experience.
Work Schedule: Sunday, Monday, Friday & Saturday: 8am – 4:30 pm. Work assignments may include holidays.
