Journey Hospice has a rewarding and fulfilling position for a per diem RN Case Manager for Atlantic and Cape May Counties.
The Hospice RN will oversee a primary caseload of Hospice patients and provide skilled professional nursing care to them, their families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards, and agency policies and procedures.
Responsibilities include:
- Providing initial and comprehensive nursing assessments and identifying patient problems.
- Collaborating to develop the Hospice Interdisciplinary Team (IDT) plan of care.
- Implementing, reevaluating, and revising the plan of care relative to the patient’s nursing needs.
- Teaching, supervising, and counseling patients, family members, and staff regarding nursing care.
- Working closely with the IDT to ensure all patient and family care needs are met.
- Providing supervision to Hospice Aides/Homemakers.
Education/Training/Certifications:
- Graduate of an accredited registered nursing program.
- Bachelor’s degree in Nursing preferred.
- Certified Hospice and Palliative Nurse (CHPN) certification preferred.
- NJ RN Licensure by the NJ State Board of Nursing in good standing.
- Valid driver’s license and reliable transportation that is insured in accordance with NJ state requirements.
Skills and Knowledge:
- Maintain a professional attitude and demeanor; communicate effectively with patients, caregivers, colleagues, and supervisors.
- Demonstrate optimism, enthusiasm, and willingness to work constructively with health care team members.
- Interact positively with all staff, supporting the mission and vision of LSMNJ.
Experience:
- Minimum of one (1) year general nursing preferred with current hospice, home health, medical, surgical, or critical care experience.
- Experience with pain and symptom management interventions preferred.
- Ability to work within an interdisciplinary setting.
- Understanding of Hospice philosophy and principles of death/dying.
