LVN, Licensed Vocational Nurse (Per Diem)
We are looking for a compassionate and skilled Licensed Vocational Nurse (LVN) to provide excellent care to our patients in their homes. As a key player in our home health team, your role is crucial in enhancing the health and well-being of our patients by delivering high-quality nursing services. This position offers the opportunity to work per diem, with flexibility and autonomy, while still becoming an integral part of an innovative and supportive healthcare team.

Essential Duties and Responsibilities:
- Provides professional nursing care within the scope of LVN practice, ensuring patient safety and comfort.
- Conducts follow-up home health visits, evaluates patient needs, and adjusts care plans accordingly.
- Works under the supervision of Nurse Practitioners or Physicians to deliver prescribed medical treatment.
- Assists Registered Nurses in educating patients and their families about health conditions and home care techniques.
- Performs basic medical tasks such as changing bandages, administering prescribed medication, and collecting specimens.
- Helps patients with daily activities such as bathing, feeding, and dressing, and monitors vital signs.
- Regularly communicates with the supervisory team about changes in the patient's condition or emergencies.
Minimum Position Qualifications:
- Active Clinical Experience for at least one year.
- Current LVN license valid in the State of California.
- Valid driver’s license with a clean DMV record.
- High school diploma or equivalent required.
- Excellent communication and interpersonal skills to effectively interact with patients and team members.
- Ability to work independently and manage time effectively.
- Basic computer skills for documenting patient care.
Preferred Qualifications:
- Additional certifications in healthcare field, such as CPR certification.
- Experience in home health care or similar settings.
- Knowledge of the Nurse Practice Act and other relevant regulations.
- Bilingual abilities (especially Spanish) are highly appreciated.
- Demonstrated ability to handle acute care situations effectively.
