Carolina Reserve of Hendersonville and Laurel Park, communities of Navion Senior Living located in Hendersonville, NC, is seeking a Registered Nurse (RN) to join the team! The RN Case Manager is responsible for overseeing the Resident Services Department in their assigned communities in accordance with state and local standards, guidelines, and regulations. The RN Case Manager will work with the Resident Care Director(s) and Executive Director(s) to ensure clinical compliance, oversight, and training to the communities they support.
Responsibilities
- Demonstrate positive customer service relations and community’s quality care and service standards.
- Incorporates wellness philosophy, standards, and objectives into the evaluation process for staff in the Wellness Department.
- Provides support and training to Resident Care Directors and Executive Directors in the community.
- Completes and assists with health assessments and updated service plans for each resident.
- Provides direction as to format and approach to resident care/service management.
- Ensures community compliance with medication management procedures in accordance with state and company guidelines.
- Serves on Quality Improvement Committees that include Resident Care management, Infection Control, and Safety.
- Ensures Resident Compliance Trackers remain compliant.
- Ensures Case Management Dashboards are updated weekly and Comprehensive Resident Review Meetings are conducted monthly with notes reflected in the dashboard.
- Supports in the discussion of high-risk residents as indicated on the dashboard or through other methods.
- Serves as the Infection Control Coordinator following all OSHA safety guidelines.
- Provides orientation and education programs for Resident Services personnel and ensures that all personnel can demonstrate competency required to function appropriately in their positions.
- Routinely schedules and conducts meetings with Resident Care Directors to communicate and operationalize objectives, standards, policies, and procedures.
- Ensures compliance with all regulatory agencies by continually monitoring the operation of the department and making changes as needed.
- Participates in the selection of residents to move into the community by evaluating the level and number of care/services required by prospective residents.
- Routinely visits the assigned communities following the schedule set by the supervisor.
- Supports in investigations of incidents and accidents in accordance with state regulations and company policy.
- Implements Infection Control policies and procedures as outlined in the company program.
- Other duties assigned by supervisor.
Minimum Requirements
- Graduate of an accredited School of Nursing and RN degree.
- Two years’ experience in an assisted living environment desired.
- Three years’ experience in a position with management and leadership responsibilities.
- Basic knowledge of and ability to apply Geriatric Nursing principles.
- Knowledge of state-specific rules and regulations.
- Must be able to handle conflict, problems, and emergency situations.
- Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary setting with residents, families, staff members, representatives of community and government agencies.
- Computer skills such as Excel and Word documents required.
- High energy level and the ability to handle multiple priorities as required.
- Ability to plan, organize, develop, and manage the objectives of the Wellness Department to provide quality care and services to all residents.
