Longleaf Hospice and Palliative Care is locally-owned with offices located in Atlanta, Covington and Roswell, offering end-of-life care to those with life limiting illnesses in the communities we serve. This position involves patient assessment, carrying out the established individualized plan of care and communication with physician to meet patient/caregiver needs: The PRN RN will:
- Use health assessment data to support the Plan of Care established by the RN Case Manager.; inform the patient and the family in the planning process
- Maintain current, accurate, detailed reports in accordance with policy
- Record patients' medical information and vital signs. Regularly reevaluate patients' nursing needs when assigned to the patient
- Monitor, record and report symptoms and changes in patients' conditions
- Prepare clinical notes and update the primary physician when necessary; obtain/receive physician orders as required
- Counsel the patient and family in meeting nursing and related needs
- Provide health care instructions to the patient as appropriate per assessment and plan
- Participate in on-call duties
- Other duties as assigned
Requirements
- Current license to practice as a registered professional nurse in Georgia
- Current and continued CPR certification
- At least one or two years of recent home care experience; hospice experience highly preferred
- Good verbal and written communication and problem-solving skills
- Responsible valid driver’s license
- Reliable, insured, automobile for making home visits required
