As part of a multidisciplinary team to deliver patient-centered nursing and rehabilitation services to patients, you will support the Unit Manager to maintain excellent clinical standards and patient experience. You will be responsible for ensuring all assigned residents receive all medications, treatments, and nursing care as ordered, and for completing all required documentation as per state/federal guidelines.
Responsibilities
A day in the life of a Licensed Practical Nurse at Hackensack Meridian Health includes:
- Supervising and making assignments of CNA’s on the unit.
- Following the nursing care plan and utilizing nursing diagnosis for patients in cooperation with the supervisor and nursing personnel on the unit.
- Consistently, accurately, and independently prioritizing and completing routine assignments within guidelines and schedules.
- Recognizing psychosocial needs, their possible effects on behavior, and intervening to provide emotional support.
- Maintaining accurate informative records on all patients in the unit.
- Identifying and reporting all real or potential problems regarding the development or administration of patient care.
- Recording, reporting, and interpreting patient responses to treatment and care to appropriate members of the inter-disciplinary team.
- Identifying any changes in patient conditions/needs and reporting to the Charge nurse.
- Demonstrating a team approach to problem-solving through frequent communication and collaboration on complex issues to provide appropriate care.
- Maintaining all equipment as per facility policy and using equipment consistent with equipment specifications.
- Participating and representing Harborage and Nursing staff on committees, as requested by supervisors.
- Identifying patient needs and treatment opportunities, providing services to meet those needs on an ongoing basis.
- Maintaining professional competence and keeping current with Occupational Health trends through participation in continuing education, in-service programs, professional staff sessions, and training of nursing personnel.
- Fostering good relationships and communications with all team members, patients, family/significant others, and visitors.
- Assisting with new team member orientation and providing key experiences to prepare team members for independent performance of essential job functions.
- Reporting problems to the manager promptly.
Qualifications
Education, Knowledge, Skills, and Abilities Required:
- Completion of an approved LPN program.
- Current with all Continuing Education (CE) requirements.
- Ability to perform clinical care professionally.
- Serve as a patient advocate and role model to other team members.
- Deliver patient care with minimal disruptions and obstacles.
- Ability to effectively work on a multidisciplinary team.
Education, Knowledge, Skills, and Abilities Preferred:
- Computer skills, including Microsoft Office.
- Long-term care facility experience.
- Prior work experience with geriatric and disabled populations.
- Bi-lingual (Spanish).
Licenses and Certifications Required:
Licenses and Certifications Preferred:
- BLS Basic Life Support.
- CPR Cardiopulmonary Resuscitation.
- IV Certification.
