JOB PURPOSE:
Provides pharmaceutical services to patients receiving care in post-acute, long term, or retail settings in a manner that delivers superior patient care and excellent customer service. Monitors and manages the prescription process and daily activities of the closed-door long-term care pharmacy in accordance with company, state and federal policies, laws and regulations.
ESSENTIAL JOB FUNCTIONS, DUTIES, AND RESPONSIBILITIES:
- Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients needed.
- Evaluate prescriber medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Make recommendations for medication therapy changes to prescribers as appropriate.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other members of the healthcare team.
- Take responsibility for accurate intake and efficient fulfillment of prescription orders.
- Obtain, as necessary, and maintain all required documentation in accordance with federal and state regulatory agencies and Pharmacy Services policy.
- Provide and document interim Medication Regimen Reviews for all new admit or readmit patients.
- Implement and document all approved therapeutic substitution medication orders.
- Provide appropriate clinical assessment prior to contacting healthcare center.
- Demonstrate effective and professional leadership skills.
- Document and investigate errors and variances.
- Take ownership and accountability for the successful completion of all pharmacy production duties and achievement of pharmacy location goals.
LICENSURE, CERTIFICATION, EXPERIENCE AND EDUCATION REQUIREMENTS:
- Graduate of ACPE accredited School of Pharmacy.
- Current licensure in the state in Georgia.
- Long-term care pharmacy experience preferred.
- Experience in patient care and medication management, drug therapy monitoring, drug and medical supply procurement, inventory control management, and performance improvement.
- Ability to clearly communicate medication information to professional practitioners and the general public.
