The Case Management Manager reports to the Director of Case Management. Under general supervision, the Case Management Manager provides professional supervision and direction to care team members in the Case Management Department (RNs, MSWs, Program Coordinators) to achieve the best clinical outcomes for patient groups. This position serves as a clinical expert and resource related to Case Management, discharge planning, and care transitions. Position Purpose

This position serves as a clinical expert and resource related to Case Management, discharge planning, and care transitions. This position has a team of case managers and/or social workers who report directly to them.
Requirements (Education, Work Experience, Licensure, Registry &/or Certifications)
Bachelor's degree in Nursing and five years’ case management work experience required; Master’s Degree Preferred; Supervisory experience preferred. Licensure by the South Carolina Board. Case Management Society of America (CSMA) or American Case Management Association (ACMA) certification preferred. Must obtain certification within 1 year, if not currently certified.
