We
are seeking a compassionate and dedicated Nurse Practitioner or Physician
Assistant to join our innovative Mobile Care Program. This unique role involves
delivering high-quality medical care to elderly patients within cluster care
facilities across the community, utilizing a fully equipped mobile unit. You
will play a critical role in improving access to healthcare for vulnerable
populations, enhancing their quality of life, and providing comprehensive care
in a collaborative environment.

Bronx and Brooklyn, NY
Key
Responsibilities:
- Patient Care: Provide
assessments, diagnoses, and management of acute and chronic medical
conditions for elderly patients.
- Mobile Outreach: Travel to
designated cluster care facilities in a mobile unit to deliver care,
ensuring patient needs are met in a timely and efficient manner.
- Health Screenings: Conduct
routine health screenings, vaccinations, and wellness checks to promote
preventive care and early detection of health issues.
- Care Coordination: Collaborate
with PACE interdisciplinary teams, including primary care physicians,
nurses, social workers, and facility staff, to develop and implement
individualized care plans for patients.
- Patient Education: Educate
patients and their families on health management, medication adherence,
and lifestyle modifications to enhance overall well-being.
- Nursing Background preferred but
not required.
- Documentation: Maintain accurate
and timely medical records in compliance with organizational policies and
regulatory requirements.
- Emergency Response: Respond to
medical emergencies and provide appropriate care or interventions as
needed.
- Community Engagement: Participate
in community outreach efforts to raise awareness of the program and
promote health services available to the elderly population.
Schedule:Full-time
40 hours per week, 8am – 5pm
Compensation
& Benefits:$170,000 plus full benefits package
Requirements:
- Licensed
to practice as a Nurse Practitioner or Physician Assistant in NY.
- DEA
Certification
- Board
Certification
- Be
medically cleared for communicable diseases and have all immunizations
up-to-date before engaging in direct participant contact.
- Must
have a Medicaid Provider ID and a Medicare Provider ID in good standing.
- Minimum
of one year of experience working with a frail or elderly population, or
the ability to receive appropriate training from the PACE organization
- Minimum
of two years of clinical nursing practice in home care, care management,
or a related field
- Effective
oral and written communication and interpersonal skills
- High-level
analytical and research skills
- Ability
to manage multiple projects and meet deadlines
- Basic
computer skills, including proficiency in Microsoft Office
