Job Description:
Provides support for clients and works collaboratively with Case Managers in Housing First models to help individuals with serious mental illness and/or substance use disorders maintain housing after experiencing chronic homelessness. Portfolio is TBA, could be a combination of multiple program teams, mix of congregate site and scattered site.
Key Responsibilities:
- Build rapport, act as a role model, and empower clients to move forward.
- Work closely with the team to link clients to services and help navigate and access SOAR Disability benefits when appropriate.
- Facilitate life skills groups and social activities.
- Record services and notes in HMIS and EPIC (electronic health records).
Qualifications:
- HS Diploma or GED preferred.
- Computer skills essential.
- Active credential for Peer Recovery Specialist or Community Health Worker required.
- PRS candidates ideally have lived experience in recovery for substance use with 5+ years of sustained recovery, as well as lived experience of homelessness.
- Prior experience working with clients experiencing homelessness ideal.
- Must have TX driver’s license with 3-year safe driving history.
