As an Aging Care Manager 1, you will be responsible for gathering information from clients, their family members, neighbors, and other individuals who may be able to provide insight into the client's needs. This information will be used to identify any social, economic, emotional, health, or physical issues that the client may be facing. In addition, this role involves helping clients access a range of services and resources. Other duties may include providing information and referrals, conducting home visits, counseling clients, and assisting in resolving any issues that may arise. An ideal candidate for this position will possess strong communication and problem-solving skills to effectively support and advocate for our clients.
Work Schedule and Additional Information:
- Full-time employment (37.5-hour work week)
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- Three years as an Aging Case Aide 2; or
- A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.
- Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree/credits.

- Hiring preference for this vacancy may be given to candidates who live within Clinton, Columbia, Dauphin, Lycoming, Montour, Northumberland, Schuylkill, Snyder, or Union counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.

- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
