ESSENTIAL DUTIES
- Provide geriatric medicine consultative services, to include:
- Inpatient consultations, follow-up encounters as appropriate
- Family meetings as required
- Assessment of capacity for medical decision-making/with support from attending physician
- Participation in team huddle during weekdays
- Participation in multidisciplinary rounds
- Collaboration with consulting teams, case management, nursing, primary care physicians, rehab physicians and hospice
- APP must provide patient care for 36 clinical hours/4 admin hours per week
- Provide community and multidisciplinary education regarding geriatric medicine; including end of life decision making, management of geriatric syndromes and symptom management, emphasizing pain control.
- Provide education and direction to medical students and residents that may be on service.
- Participate in regional/community events in collaboration with public relations department including participation in regional committees that would facilitate the dissemination of education throughout the community.
- Provide geriatric medicine education to primary care and inpatient providers and serve as a resource for questions regarding specific care plan.
- Participate in Munson Health, system geriatric care related meetings and educational opportunities, collaborate with Munson Health System in regard to operations of the geriatric service line.
- Maintain a working knowledge of applicable Federal, State and local laws and regulations, hospital and corporate compliance, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
- Participate in quality improvement activities as appropriate/assigned and report results on a routine basis to appropriate medical quality improvement programs.
- Participate with planning and implementing operational decisions, policies and procedures relative to geriatric medicine.
- Cooperate with Munson in marketing geriatric medicine services.
- Provide input regarding equipment needed, providers and standards required and other appropriate process ease, and communicate these to appropriate administrative staff.
- Actively work with Munson to ensure patient flow and access, monitoring and controlling cost, in utilization of services.
- Participate with management, planning and implementing programs to enhance services.
STANDARD QUALIFICATIONS
Education / Training
- Master's Degree in Nursing OR a Masters in Physician Assistant studies.
Certification, Licensure, Registration
- State of Michigan Nurse Practitioner/Physician Assistant license required.
