We are seeking a compassionate and skilled Registered Nurse (RN) to join our home health team. In this role, you will deliver one-on-one patient care, ensuring the highest standards of health and well-being for our clients. Job Summary
The role of the RN Home Health: Clinical Referral Specialist is to act as the clinical liaison that represents the agency for referrals to home health.
- Acts as the Intake clinical liaison that represents the Agency via telephone with referrals for patient care from physicians, hospitals, insurance case managers and all other referral sources.
- Responds to phone calls, electronic transmissions, e-mails, and faxes regarding clinically complex referrals with an extremely customer-friendly, service orientation at all times.
- Accurately completes referral documentation as necessary to facilitate the admissions process.
- Reviews physician orders and follows up with referral source for clarifications or additional information if needed.
- Identifies objections, concerns or other entry barriers and provides information in response to address those issues. Proactively offers alternatives if the Agency is unable to provide services.
- Processes and documents referrals that are not accepted for admission, indicating specific reason for non-acceptance, according to policy and procedure.
- Participates in the development of policies and procedures and actively assists in problem solving with and between departments to ensure the clinical intake function is collaborative and effective.
- Maintains positive professional relations with support personnel through appropriate and timely responses for information or paperwork necessary for key clinical and billing functions.

Qualifications
Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's degree in nursing preferred.
Minimum of one year of professional nursing experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required. Basic Life Support (BLS) required. Certificate for Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) preferred.
