Job Summary and Qualifications
As an Advanced Clinical Pharmacist, you will demonstrate proficiency in all pharmacist functions by attending and actively participating in daily multidisciplinary and bedside rounds, completing drug utilization reviews, leading and developing clinical services, and mentoring staff to ensure accurate documentation of clinical activities. You will conduct drug regimen reviews and respond to specific polypharmacy, pharmacokinetic, and medication/MAR reconciliation consults.
What you will do in this role:
- Interpret orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system.
- Accurately compound and dispense pharmaceuticals.
- Comply with facility policy and procedures and regulatory requirements.
- Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
- Provide clinical consultation and clarification to practitioners.
- Sustain the hospital formulary, minimizing nonformulary procurements, and promote rational drug therapy selection.
- Attend and actively participate in daily multidisciplinary and bedside rounds.
- Provide accurate and timely medication information to healthcare staff and patients.
- Participate in pharmacy clinical services such as drug consults, nutrition support, and other programs as defined by Pharmacy Leadership.
What qualifications you will need:
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy.
- Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience required.
- Completion of hospital PGY1 / pharmacy general practice residency or fellowship.
- BCPS certification or other Board of Pharmacy Specialty (BPS) certification.
- Current Registered Pharmacist licensed by the Texas State Board of Pharmacy or licensed pharmacist in another state.
