Position Summary:
Under the direction of the San Diego PACE Medical Director, the PACE Physician provides comprehensive medical care to the PACE participant. The PACE Physician attends to the acute needs of the PACE participant, while managing chronic medical problems and paying attention to the preventive and rehabilitative care required by the PACE participant. The PACE Physician incorporates the entire interdisciplinary team as appropriate to provide the medical care in a holistic manner, as is the hallmark of the delivery of care to the PACE participant.
Essential Functions of the Job:
- Is responsible for prescribing appropriate medication, health regimens, and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues, and promote a healthier life.
- Is comfortable managing stable patients with chronic conditions by utilizing internal resources, such as e-consult services, and other references to maintain their general health without the need/urge to refer patients to outside specialty care providers.
- Is able to triage and quickly recognize when treatment is appropriately provided on site versus in the urgent care or emergency room.
- Responds to and provides crisis intervention services when needed; ensures behavioral health services are delivered in a culturally appropriate fashion, and is consistent with State of California laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines.
- Refers the appropriate patient to the appropriate Behavioral Health providers including: the Psychiatrist, the Psychiatric PA, the Psychiatric NP, the Doctorate in Psychology, the Licensed MFT team, and other appropriate BH members providing these services.
- Works in conjunction with the Nurse Practitioner and the Physician Assistant advanced mid-level medical providers who play an integral role in the delivery of primary care to the PACE participants.
- Participates in the supervision of the advanced mid-level provider as required by San Ysidro Health practices.
- Will supervise family practice and/or internal medicine residents that are enrolled in our FP/IM residency programs.
- Efficiently and effectively prioritizes daily tasks and duties that help maintain high-acuity patients out of emergency and urgent care facilities and/or enables safe and expedient transitions of care.
- Completes all required medical documentation in the Electronic Medical Record in a timely manner consistent with excellent documentation in compliance with San Diego PACE requirements, and State and Federal regulations.
- Assists in developing and implementing procedures and protocols to facilitate the delivery of comprehensive health services and quality assurance.
- Provides inpatient coverage and call coverage as assigned by Medical Director.
- Screens and monitors the management of chronic conditions, symptomatic and well patients.
- Coordinates the preparation of reports and written documentation including medical records, as required by the department clinic, state and federal agencies.
- Supervises Nurses and assistants in examining room and may serve as preceptor for team members developing in expanded medical roles (Physician Assistant and Nurse Practitioner).
- May provide consultative services for complex diagnostic or therapeutic problems as requested by other staff physicians.
Additional Duties and Responsibilities:
- Maintains and updates established departmental policies and procedures as needed, objectives, quality assurance program, safety, environmental and infection control standards.
- Enhances professional growth and development through participation in educational programs, in-service meetings and workshops. Remains current in literature pertaining to primary specialty.
- Attends meetings as required and participates in committees as directed.
- Performs other duties as assigned.
Job Requirements
Education Required:
Minimum:
- Graduate from accredited school of Medicine.
Certification/registration:
- Current CA MD and DEA license.
- BLS certification.
- Board certified or eligible in primary care specialty.
Experience Required:
- Completion of clinical training.
Supervisory Experience:
Verbal and Written Skills Required to Perform the Job:
- Good oral and written communication.
Technical Knowledge and Skills Required to Perform the Job:
- Preferred skills include skin biopsy, excision of skin lesions, suturing, casting, and sigmoidoscopy, phlebotomy.
- Must obtain staff privileges at local hospitals.
Equipment Used:
- Diagnostic and therapeutic: spirometry; EKG; peak flow.
Working Conditions and Physical Requirements:
- Prolonged periods of standing and walking.
- May assist patients in getting onto examination table.
- Will be required to work at other sites.
- Will be required to participate on on-call panel and work evenings and/or weekends.
Universal Requirements:
