About Twin Cities Physicians: Established in January 2016, TwinCities Physicians now serves over 4,000 patients within 200+ senior living communities across the State of Minnesota. We take a coordinated and collaborative approach to keeping residents healthier longer and safer in their communities, helping to avoid unnecessary ER and hospital visits. Our goal is to alleviate some of the burden and stress that senior communities and families feel when trying to coordinate needed medical services for residents and their loved ones.
Position Summary: Our goal is to be our patients' primary care provider through the rest of their life, by creating long-standing relationships with our patients in order to help them live as healthily as possible. With TwinCities Physicians, you'll provide a personalized touch in guiding your patients' care plan, bringing our services to all types of senior living communities.
Responsibilities:
- Provide on-site primary care services, completing 12-18 daily visits, to patients in senior living settings.
- Establish and maintain trusting relationships with your patients across their care continuum.
- Collaborate with APPs to build care plans, while offering recommendations and teachings for more effective care.
- Maintain patient care in place; reducing polypharmacy and avoiding patient hospitalizations.
Qualifications:
- Unrestricted Physician license through the MN Board of Medical Practice is required.
- Passion for, and knowledge of, the Geriatric population best care practices.
- Prior experience rounding in Senior Living environments, or as a Hospitalist, would be beneficial.
- Strong communication skills are necessary for communicating with patients and community health staff.
- Valid Driver’s License and current Auto Insurance are necessary.
