MEDICAL SOCIAL WORKER (MSW) - HOSPICE
LOCATION: SOUTH CITY/SUBURBS
HOURS: DAYS M-F 8am-4:30pm. Must be able to work 1 summer and 1 winter holiday and rotate in on call schedule.
This position is also eligible for up to a $5,000.00 sign on bonus! Internal team members are not eligible for this sign on bonus at this time.
JOB and UNIT SUMMARY:
The purpose of this position is to help achieve the vision of Advocate Hospice Services by performing the duties of this position in a cost-effective, results-oriented, and customer-focused manner. This position is a critical component of the interdisciplinary team, and meeting individual performance expectations and contributions to team performance goals is required.
- Responsible for the development and enhancement of the psychosocial and counseling support services provided by hospice in collaboration with other health care providers and members of the hospice interdisciplinary team.
- Participates in the QA/CQI process for Advocate Hospice.
- Responsible for achievement of agency, department, and personal goals and objectives.
- Responsible for the assessment of psychosocial needs of patients and families as well as development of a social work plan of care reflecting problems, interventions, and outcome goals. Implements the plan of care in conjunction with the interdisciplinary team.
REQUIRED QUALIFICATIONS:
- Masters degree in social work from an accredited school of social work.
- Minimum of one year experience in a medical clinical setting and/or hospice/palliative care.
- Social Work Licensure in the state of Illinois (LSW).
- Experience working with extended families during times of excessive stress, pending deaths, and during the process of active dying of loved ones.
COMPENSATION: $61,568-$92,352.00 annual base salary.
