Objective/Overview
CUHCC’s mission is to seek health equity in our community by advancing the well-being of diverse people. The clinic is known for providing high quality care to diverse patients and is moving forward in improving timely access to care for patients. This position provides mental health therapy and assessment services to patients in an outpatient setting as part of the mental health outpatient therapy team. The position has independent responsibility and accountability in clinical decision-making and performing the duties of an independently licensed mental health professional as defined by MN statute.
A qualified candidate for this position will be hired into one of three job codes depending on licensure. With the exception of licensure, essential functions and minimum requirements are the same across all three job codes. The job codes and licensure requirements are as follows:
- Licensed Professional Clinical Counselor (LPCC), and be placed in a 9721CG (HC Pro 5 Counseling) job code
- Independent Clinical Social Worker (LICSW) and be placed in a 9721SW (HC Pro 5 Social Work) job code
- Licensed Marriage & Family Therapist (LMFT) and be placed in a 9721MF (HC Pro 5 Marriage & Family Therapy) job code
Primary work environment is at the CUHCC clinic during clinic hours. This role may come into contact with patients who may have contagious illnesses. Telemedicine remote work may be available 1-2 days a week based on patient need/demand and productivity expectations.
Job Responsibilities
Therapy and Assessment Services (100%)
- Assess, diagnose and treat patients, develop patient care plans based on defined areas of clinical expertise, risk and overall patient care complexity.
- Conduct diagnostic assessments, treatment planning, individual, group, couple and family therapy interventions.
- Prepare and complete mandated documentation within established timelines including diagnostic assessments, individual treatment plans, progress notes, discharge summaries, safety assessments, and required screenings (CASII/ECSII, PHQ-9, CAGE, etc.).
- Maintain daily productivity expectations as defined for outpatient therapist.
- Identify desired patient outcomes, evaluates and monitors patient’s outcome and response to care, using current best practice guidelines and developed CUHCC clinical protocols in a cultural context.
Required Qualifications
- Completed a Masters or Doctorate degree program in counseling, social work, marriage & family therapy or a related field.
- Master’s degree plus at least four years of experience, or a terminal degree plus at least one year of experience.
- Independently licensed Mental health Professional as defined by MN statute as a Licensed Professional Clinical Counselor (LPCC). License must be active and in good standing.
- Document patient care in the electronic health record, communicating significant changes in patient status according to established clinic procedures and timelines.
- Contribute to the efficiency and effectiveness of the unit’s service by participating as an active member of the patient care team in an integrated setting.
- Coordinate services with the client’s other service providers, such as mental health case manager, ARMHS worker, psychiatry provider, school team, upon consent of the client.
- Meet mandated annual requirements for continuing education per the professional’s licensing board.
Preferred Qualifications
- Experience in providing clinical services to immigrant, refugee or other underserved populations in a multicultural practice setting.
- Fluency in Spanish, Somali, French or any South East Asian language.
- Experience and training in Trauma Informed Care.
