Overview
The incumbent ensures efficient disease management efforts to improve patient outcomes and patient satisfaction. The incumbent is responsible for performing initial and ongoing needs of the patient: maintaining frequent patient contact for disease specific programs and implementing appropriate interventions as prescribed. The incumbent coordinates the development, implementation and monitoring of the patient's plan of care by collaborating with the physicians and other members of the medical care team. The incumbent identifies patient educational needs and provides or coordinates ongoing education to help improve patient health outcomes.
Qualifications
Required Education:
Bachelor's degree
Specific Course Work:
Graduate of an accredited school of nursing
Preferred Education:
Master's degree
Necessary Skills:
- Organized and self-motivated.
- Able to work independently.
- Possesses assertiveness skills and above average verbal and written communication skills.
- Knowledgeable in principles of adult education.
- Excellent interpersonal skills with patients and families, staff, physicians, other disciplines and administration.
- Computer skills including Power Point, Word, Excel.
Required Licensure/Certifications:- Licensed to practice nursing in the state of Florida.
- BLS from the American Heart Association.
- All new hires or transfers must have a professional certification within two years of hire or transfer. For certifications that require a specified number of cases or hours worked within the specialty, the certification must be applied for once the case/time requirement has been met, and the certification must be obtained within one year of the requirement having been met (e.g. oncology, chemotherapy, diabetes education).
- Certifications as required for assigned units. Exception - Diabetes Educators are required to conduct 2000 patient teaching hours before they are eligible for certification. Therefore, Diabetes Educators must have Diabetes certification within 4 years of hire.
Preferred Licensure/Certifications:ACLS from the American Heart Association is required for the ICU, Emergency Department, and Specialty areas.
Required Experience:3-5 years of Medical/Surgical RN experience.
