Job Description
PsychPros is partnering with a well-respected community mental healthcare organization focused on increasing the stability of clients' lives by providing resources for safe housing and developing plans to help maintain independent living.
Job Duties Include:
- Maintain caseload of up to 35 program participants.
- Regularly meet with program participants, at a minimum on a monthly basis, in the office or off-site to review program participant budget and bill payment.
- Regularly meet with program participants to discuss and review issues that impact program participant income and housing, and review program participant case plans for stability and self-determination.
- Mandatory bi-monthly home visits with each program participant and in the month prior to HUD recertification to ensure compliance with HUD requirements.
- Coordinate program participants' access to government services and other non-profit services.
- Advocate for program participants when necessary, and empower program participants to advocate for themselves when appropriate.
- Maintain accurate and up-to-date documentation of program participant case plans, case notes, change of status documents, and other reporting and tracking forms.
- Provide essential services necessary to reach out to unsheltered homeless people: connecting them with emergency shelter, housing, or critical services.
Support Coordinators/Case Managers will work with a team of dedicated social service employees and provide respectful and collaborative services to clients.
Case Management Requirements
- Bachelor's degree
- Minimum of 1 year experience working within a social service setting.
