Compensation Range: $135k - $140k annually commensurate with experience.
Overview:
The Nurse Practitioner provides comprehensive medical evaluation, education, and management for clients that present for routine, preventative, acute, and chronic disease health care services at the clinic site. The nurse practitioner will coordinate all of the services necessary for health promotion, maintenance, rehabilitation, and the prevention of disease and disability.
Responsibilities:
- Assesses and interprets medical and psycho-social histories; performs physical examinations (including gynecological and breast examination as appropriate); identifies problems; formulates a plan of treatment; provides education and referral (as indicated); and evaluates the effectiveness of the interventions prescribed.
- Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral course when indicated.
- Performs or orders laboratory tests and interprets the results as part of the health management. Reports communicable disease in a timely manner to the appropriate Health Department as per health center protocol.
- Follows established treatment guidelines as indicated by Health Center protocols.
- Conducts health teaching and counseling during the medical encounter, as appropriate, enabling the client to make informed choices about health and treatment options.
- Completes the client’s clinical record documentation in a timely manner specific to the applicable policies and procedures of the health center.
- Understands the medical emergency procedures and is familiar with the location of the emergency kit/supplies in all clinic settings.
- Participates in quality improvement/assurance activities.
- Participates in continuing medical education activities to enhance professionalism and to ensure quality of care.
- Provides on-call coverage as scheduled.
- Works collaboratively with all members of the health center to build a supportive team.
- Applies infection control measures according to Health Center Policies and Procedures.
- Maintains professionalism and adheres to client confidentiality.
Minimum Requirements:
- 2 to 4 years of experience as a Nurse Practitioner.
- Experience and knowledge of HIV/AIDS primary care preferred.
- Experience and knowledge of managing clients with substance abuse preferred.
- Licensed to practice as a NYS Nurse Practitioner.
- Licensed and currently registered as a Registered Nurse in the State of New York.
- Previous experience with an electronic health record, preferred.
- Visual acuity and manual dexterity to interface with the computer and perform medical procedures as indicated.
- Auditory acuity to handle phone calls and interact extensively with patients.
- Maintains CMEs/CEUs required for licensure.
- Certified by ANCC or AANP.
- Bilingual English/Spanish preferred, but not required.
