Nurse Practitioner/Physician Assistant Primary Care
Job Summary
The Nurse Practitioner/Physician Assistant will provide primary care services to patients in an assisted living/skilled nursing facilities. The successful candidate will work collaboratively with the facility's medical director and other healthcare professionals to provide high-quality care to patients.
Responsibilities
- Conduct initial and ongoing assessments of patients' health status
- Develop and implement care plans based on patients' needs and goals
- Prescribe medications and treatments as appropriate
- Order and interpret diagnostic tests
- Provide patient education and counseling on health promotion and disease prevention
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care
- Maintain accurate and up-to-date medical records
- Participate in quality improvement initiatives
Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky
- Master's degree in Nursing or Physician Assistant Studies
- Certification as a Nurse Practitioner or Physician Assistant
- Experience in primary care, preferably in an assisted living or long-term care setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
