JOB TITLE : LPN
DEPARTMENT: Nursing
REPORTS TO: Practice Manager
FLSA (TBD by HR): Full time, Non-Exempt, Union
JOB PURPOSE: The Licensed Practical Nurse (LPN) will work as a member of the primary health care support team to provide quality medical aid to patients in the Family Medicine Department and may have to float to the Pediatric and Obstetrical Departments. The LPN will adhere to standards and policies established by AJHC and perform the duties up to limits prescribed by State Licensing Regulations.
MAJOR RESPONSIBILITIES (Essential functions):
- Documents the principal complaint or reason for the visit, including vital signs and appropriate screenings (vision, hearing, UA, Pregnancy test, EKG).
- Administers medication and performs various procedures during patient visits, as prescribed by the provider.
- Provides medical education to patient/clients on medications and follow-ups.
- Reviews medical records for completeness of data prior to patient/client visit, including:
- Scans the provider’s last note to determine if lab data/results have been received.
Procures lab data/results/information prior to patient appointment. Reports to the provider any significant social or other information obtained during conversation with patient or his/her family. Works with the Provider to actively identify and detect conditions and to provide high quality care to patients/clients and families. Maintains an adequate supply of medical, nursing, and laboratory supplies and orders as needed on a routine basis. Alerts appropriate personnel when there are mechanical failures of equipment. Keeps exam rooms and work areas clean and well stocked according to established standards. Completes the paperwork required to transport laboratory specimens obtained during the clinical visits. Completes the paperwork required to transport patient/client to ED by ambulance or car. Maintains client confidentiality and protects operations by keeping information confidential as per HIPAA regulations. Contributes to team effort by accomplishing related tasks as needed. Uses technology and office resource tools to perform the job.
Punctual and custom attendance are required to maintain effective relationships with team members and patients/clients.
LPN will gather data and records as required from outside resources (RHIO, iStop, etc.).
LPN will assist in reviewing patient’s records for any gaps in care related to measurable goals and appropriate patient care.
WORKING CONDITIONS:
The working conditions of the position are in a typical clinic and office setting. Clinic and office settings are indoor and predominantly located in inner-city locations.

The mental demands for this position include: multi-tasking, ability to concentrate, balancing competing priorities, alertness, etc. High level of focus to detail, requires alertness, ability to balance competing demands, ability to concentrate, keep calm under pressure and manage deadlines.
MACHINES OR EQUIPMENT:
The jobholder is responsible for operating: phone, fax, copier, computer, and medical equipment.
SUPERVISION:
The jobholder’s work is reviewed by: Practice Manager and Clinical Chief.
EDUCATION AND EXPERIENCE REQUIRED:
Accredited LPN program.
LICENSES AND CERTIFICATIONS:
- Current NYS LPN License without any negative actions and certification to administer medications.
- Current Provider BLS/CPR through the American Heart Association.
- Infection Control Certificate.
