We are searching for qualified candidates to fill our Nurse Practitioner or Physicians Assistant role.
The correct candidate will have at least 1 year of experience performing the following duties:
- Assessing patient needs while delivering quality care and treatment to achieve optimal patient outcomes for patients with chronic kidney disease and ESRD.
- Monitoring and documenting patient condition on an ongoing basis and responding to the care plan.
- Communicating with the Nephrocare team about patient conditions or updates on a consistent basis.
- Assuring that patient needs for immediate and/or emergent care are handled promptly.
- Collaborating well with all levels of the clinical team.
- Understanding value-based care and principles to practice.
- Willingness to slow progression and minimize complications while keeping the patients' ideals in mind.
