The role you’ll contribute:
The Social Work Care Manager intervenes with patients who have complex psychosocial needs, requiring assistance with eligibility determination for social programs, funding sources, and community assistance from various special assistance programs and agencies. This role also provides crisis intervention to patients and families with psychosocial needs and coordinates the development of discharge plans for high-risk patient populations. The Social Work Care Manager collaborates with the patient/family, care manager nurses, nurses, physicians, and the interdisciplinary team to ensure patient-centered care coordination throughout the continuum of care. The role is responsible for patient evaluations of post-hospital needs, developing and implementing transitions of care plans prior to patient discharge, and ensuring optimal patient flow and continuity of care. The Social Work Care Manager communicates daily with the interdisciplinary team during multidisciplinary rounds and actively participates in outstanding customer service, maintaining respectful relationships with all.

- Receives referrals for psychosocial complex needs from the healthcare team.
- Provides assessment and reporting interventions in cases of child abuse/neglect, domestic violence, adult/elderly abuse, child protection, sexual assault, and human trafficking as appropriate.
- Provides consult services for patients who may lack decision-making capacity, coordinating with Care Management leadership throughout the guardianship process.
Qualifications:
The expertise and experiences you’ll need to succeed:
- Masters in Social Work (MSW)
- Minimum three (3) years experience in hospital/medical social work
