Job Title: RN Case Manager (Hospice RN)
Job Summary:
We are seeking a compassionate and experienced RN Case Manager to join our hospice team. The RN Case Manager will be responsible for coordinating and providing quality care to patients with terminal illnesses, ensuring they receive the highest level of comfort and support during their end-of-life journey.
Responsibilities and Duties:
- Conduct initial assessments and develop individualized care plans for each patient.
- Monitor and evaluate patient conditions and provide skilled nursing care.
- Coordinate with interdisciplinary team members to ensure comprehensive care.
- Educate and support patients and their families on end-of-life care options.
- Work closely with physicians and other healthcare professionals to manage pain and symptoms effectively.
- Advocate for patient preferences and assist with advanced care planning.
- Maintain accurate and timely documentation of patient care and progress.
Qualifications and Skills:
- Valid RN license in the state of practice.
- Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and empathetic approach to patient care.
- Knowledge of end-of-life care practices and principles.
