Job Summary: Provide high quality cardiovascular care to Kaiser Permanente members in a cost effective manner and in accordance with accepted professional practice standards and in collaboration with other members of the Cardiovascular Healthcare team.
Essential Responsibilities: - Review cardiac patients to evaluate for appropriateness of workup and treatment plans and to ensure the appropriate level of care is provided. Perform cardiology consultations when indicated or requested. Specific duties include but are not limited to completing histories and physicals, reviewing cardiac studies, ordering diagnostic studies and medications, providing pre-procedure education, writing admitting orders and scheduling procedures as needed. Collaborates with Cardiovascular service line.
- Provide comprehensive outpatient cardiovascular care in collaboration with the Cardiovascular Service line and other members of the health care team. This may include initial consultation/evaluation. Provides ongoing management of chronic cardiovascular disease.
- Provide outpatient clinic follow-up for patients discharged following cardiac procedures and for patients needing ongoing cardiac outpatient care as determined by a physician. Specific duties may include ordering tests and medications, ordering therapy and rehabilitation services, providing post-procedural education, writing discharge and transfer orders, and dictating discharge summaries. Rounding and responding to patient needs as determined by a supervising physician.
- Perform other duties as requested. Actively participates in message management to include physician in-basket and virtual care of the patient.
