Hourly Pay Range (Minimum - Midpoint - Maximum): $75.458 - $97.365 - $119.272
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

Required Qualifications- Other: Education pursuant to licensure requirement.
- 1 Year experience in a managed care or ambulatory care setting.
- California Pharmacist License - CA State Board of Pharmacy - REQUIRED
Preferred Qualifications- Completion of American Society of Health System Pharmacist (ASHP) Accredited managed care pharmacy residency.
- Experience with anti-coagulation and/or chronic disease clinics.
- BPS Board Certified Ambulatory Care Pharmacist (BCACP) - Board of Pharmacy Specialties - PREFERRED
- BPS Board Certified Pharmacotherapy Specialist (BCPS) - Board of Pharmacy Specialties - PREFERRED
Other Qualification Requirements- Certification in anti-coagulation, geriatric pharmacy, or medication therapy management within one year of hire.
Essential Functions- Advisory Service Activities and Outcomes:
Provides advisory pharmacy services to independent providers in alignment with the missions, values, and philosophies of Healthcare. Assists providers in complying with health plan quality metrics through literature based pharmaceutical recommendations. Creates analysis of relevant health plan metrics and outcomes data to determine trend outcomes. Provides appropriate follow through with prescribers. Consistently responds to drug information review requests promptly and professionally. Consistently participates in cost savings measures across the continuum of care, such as pharmacy quality metrics. Provides clinical education and in-services to providers, office staff and patients relating to pharmaceutical care. Promotes cooperation and collaboration between physicians, nurses, patients, and their families. Communicates effectively and efficiently to physicians, patients, and caregivers as a member of an interdisciplinary team in an advanced primary care model. - Clinical Competency:
Demonstrates thorough understanding and application of current and up-to-date evidenced based clinical practice guidelines and pharmacotherapy. Has attended core classes and met minimum certification requirements for anticoagulation, geriatric pharmacy, MTM, etc. Demonstrates strong organizational, analytical, and problem-solving skills. Uses professional judgment and clinical expertise in the daily solving of complex problems under the provision of clinical information. Demonstrates knowledge of age and disease specific dose adjustments and medication delivery systems. Identifies areas of improvement where systems modifications could result in operational advancements and efficiencies. Participates in designing, developing, and implementing pharmacy clinical programs (via Lean Six Sigma or Continuous Quality Improvement projects) to promote quality, cost-effective pharmacy benefits for patients. - Daily Operations:
Other ambulatory care management. Works well in a team and an individual contributor. Participates in advisory group meetings, focus groups, one-on-one physician meetings and/or other designated activities (typically before and after physician office hours) as necessary to provide physician education counseling and collaborative planning. Attends and actively participates in all mandatory departmental staff and organizational meetings. Meets all task and assignment deadlines or turn-around times. Readily adapts to changes in department staffing, work shift, and work location as business needs require. Frequently travels to off-site doctor’s offices, regional meetings, and advisory meetings, etc. as needed. Independently manages multiple projects with minimal supervision. Develops an organized work plan in a rapidly changing environment. Supports care management related pharmacy activities, drug therapy monitoring, and staff education with a focus on health plan quality metrics and improved health outcomes. Leaves clear and concise clinical notes in the patient's electronic health record and/or case management software. Actively participates in the training of others to perform all aspects of pharmacy service, including student training programs. Enters data correctly into computerized patient medication profile/EHR and other records as appropriate. Completes UM reviews for medication prior authorization referrals in accordance with appropriate criteria (e.g. health plan criteria, NCCN, evidence-based guidelines) and best practices within established time thresholds. Drug referrals may include but are not limited to: injectables, infusion therapy, DME, etc. Manages medication refill requests accurately and efficiently according to authorized protocols and collaborative agreement. - Medication Review and Safety:
Conducts medication review of prescription and non-prescription medications which may include but it is not limited to an emphasis on patient assessment and counseling, safe and effective use of medication, identification of potential drug interactions identification of medication related problems, assessment of medication adherence barriers, medical necessity, drug prior authorizations, and other related activities. Develops patient centered pharmaceutical care plans, including appropriate follow-up and monitoring parameters. Counsel patients on medication use consistent with professional standards. Assures patient and/or caregiver understanding of each medication's use, drug regimen and potential adverse events. Provides medication compliance tools as appropriate. Provides injectable medication referral reviews in accordance with health plan criteria and best practices. Quantitative work may reflect in completion of comprehensive medication reviews and/or targeted interventions programs. Reviews are subject to audit for clinical and performance standards.
