Alaris Health located in NJ is looking for an External Marketer/Liaison for Essex County.
The purpose of the External Marketer/Liaison is to assess and monitor an individual's status to determine appropriateness for continued medical services, as well as coordinate and evaluate continued services through communication and collaboration with individual patients, family members, and other healthcare professionals.
Duties and Responsibilities:
- Evaluate referrals from hospital social workers, physicians, case managers, patient care coordinators, and families by telephone and in-person assessment.
- Establish and maintain working relationships with referral sources.
- Advocate for clients to provide the appropriate level of care.
- Monitor ongoing medical status of patients for any changes in condition.
- Maintain and demonstrate working knowledge of the physical, psychological, psychosocial, and financial aspects of the rehabilitative process.
- Obtain accurate current and past medical history of patients.
- Collaborate with Admission Staff, Internal case manager, and Director of Nursing to ensure ease of transition for clients to the facility.
- Obtain insurance eligibility.
- Determine level of care to be administered to patients.
- Promote effective communication between patients, family member(s), healthcare professionals, and payors.
- Collaborate with patients, social workers, case managers, patient care coordinators, family, and physicians in establishing treatment goals.
- Establish contact with insurance case managers and negotiate cost-effective rates.
Minimum Requirements:
Must have at least one year of experience in healthcare in subacute/skill nursing.
