The role you’ll contribute:
The Social Work Care Manager intervenes with patients who have complex psychosocial needs, requiring assistance with eligibility determination for social programs, funding sources, and qualifying for community assistance from various special assistance programs and agencies. This role also includes providing crisis intervention to patients and families with psychosocial needs and coordinating the development of a discharge plan of care for high-risk patient populations. The Social Work Care Manager collaborates with the patient/family, care manager nurses, physicians, and the interdisciplinary team to ensure patient-centered care coordination through the continuum of care. Responsibilities include patient evaluations of post-hospital needs, development and initiation of transitions of care plans prior to patient discharge, and ensuring optimal patient flow and continuity of care. The Social Work Care Manager communicates daily with the interdisciplinary team during multidisciplinary rounds and facilitates collaborative management of patient care across the continuum, intervening to remove barriers to timely and efficient care delivery. Education on issues related to resource utilization, medical necessity, and care coordination is also provided to the team. The Social Work Care Manager adheres to departmental and system goals, objectives, policies, and procedures, ensuring quality patient care and regulatory compliance.

- Provides consult services for patients who may possibly lack decision-making capacity, coordinating with Care Management leadership throughout the process.
The expertise and experiences you’ll need to succeed:
- Masters in social work (MSW)
- Minimum three (3) years experience in hospital/medical social work
