Responsible for serving as the first reliable point of contact for Patients, Families, and Providers.
Principal Accountabilities:
- Serves as patient and family advocate for liver patients.
- Coordinates the scheduling of appointments to expedite the plan of care and to promote continuity of care.
- Schedules, re-schedules, and cancels appointments for the appropriate outpatient service.
- Assists in education of the patient/family to understand disease process and care map.
- Assists in answering patient calls, triaging patient calls, initiating call backs and follow-up to meet the needs of the patient.
- Assists with communication to internal departments and external contacts for navigating the Liver Transplant partnership with NYU Lagone.
- Assists with communication to internal departments and external contacts for insurance verification and authorization process.
- Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment.
- Performs other related duties as assigned.
