Responsibilities
- Provides clinical expertise in the nursing management of the patients, documentation, and care planning
- Processes start of care workflow once start of care visit has been completed
- Participates in IDG
- Reviews visits plotted on SOC or Re-cert visit and adjusts as necessary
- Maintains orders in HCHB
- Manages admissions, deaths, discharges as well as assists with these to make sure proper documentation is completed
- Assists in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assigned
- Upholds effective communication with referral sources to ensure smooth transition of admissions
Qualifications
- Associate degree in Nursing with current license
- Three years practice as a Registered Nurse
- Previous Hospice experience preferred
- Knowledge of the Hospice Philosophy of Care
- Ability to use Microsoft Word, Excel, Outlook, and other software programs
