Nurse Practitioner: Minimum - Registered Nurse Licensure from the state of Indiana; Nurse Practitioner certification, CPR required.
Physician Assistant: Minimum - Physician Assistant Licensure from the State of Indiana; National Commission on Certification of Physician Assistants (NCCPA) certification; CPR certification required.
