Our Lead APCs are critical to the success of the Clinical Community that provides compassionate care to the frailest elderly who have chronic and acute illnesses or injuries living in their homes. The Lead APC will have direct responsibility for APCs and SWs outcomes within their community. The Lead APC will partner with the designated community physician to drive clinical outcomes, team development, and daily needs of the staff within their community of patients. Deliver primary, urgent, and acute medical care to patients within the community and their homes. NP will contribute to and facilitate onboarding of new APCs and Social Workers, IDT Think Tanks, and structured ongoing education. You will also implement novel ways to improve clinical outcomes for the field. Lead APC's will facilitate an inclusive team culture consisting of physicians, social workers, paramedics, and care team coordinators to assist in delivering concierge care to every patient.
Responsibilities:
- Partner with Physicians to ensure the community meets utilization goals, operational and clinical outcomes.
- Root cause analysis of suboptimal patient and community resource(s) outcomes to develop care plans to achieve optimal patient outcomes.
- Collaborate with the Community Physician, NPs, Clinical Liaison, and Paramedics to meet the needs of patients transitioning from the hospital, facilities, or patients in crisis.
- Order, interpret, and analyze diagnostic tests and determine the need for follow-up appointments and further treatment options.
- Prescribe and manage medications and treatments as appropriate, including but not limited to psychopharmacological agents.
- Initiate and carry out performance improvement plans, orientation, and ongoing evaluation of APCs and SWs.
- Able to deliver and train on end-of-life and palliative care.
Qualifications:
- Expert in diagnosis and treatment of chronic and urgent medical/psychological conditions of older adults.
- Very strong leadership qualities and professional demeanor.
- Experience in home care, end-of-life, geriatrics, internal medicine, palliative care, ED, ICU, or SNFs.
- Experience in managing, training, and ongoing mentorship of APCs.
Must have experience leading other NPs and/or PAs. No COVID vaccination required.
Salary: $120,000 to $140,000 plus depending on experience, plus bonus and great benefits.
