POSITION SUMMARY:
We have an exciting opportunity to join our team as a Community Health Representative.The Beatrice W. Welters Breast Cancer Program Patient Navigators primary focus is to conduct breast health outreach and education programs in community-based venues (beauty salons, churches, mosques, etc.) , identify women who need breast screening and guide those women through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The mission of the Beatrice W. Welters Breast Cancer Program is to eliminate the unequal cancer burden experienced by minority populations through ensuring proportional representation in clinical care and research activities.
JOB RESPONSIBILITIES:
- Liaise with staff at the Laura and Isaac Perlmutter Cancer Center, the Beatrice W. Welters Breast Cancer Program and community agencies
- Formulate relationships with members of the community by visiting churches, synagogues, schools, libraries, etc. Work with community groups to increase breast health awareness, and involve the community in program planning if possible.
- Identify target population by utilizing community service data, i.e., cancer mapping and/or other pertinent methods for women at high risk for underutilization of breast health services.
- Guide patients though the health care system from screening, through diagnosis into treatment and survivorship
- Utilize interventions and strategies that are appropriate to the population, i.e. taking into account culture, language, age, gender, etc.
- Call patients with abnormal screening results to schedule diagnostic testing.
- Provide reminder calls to patients with appointments.
- Identify barriers to care and mitigate (e.g., transportation).
- Connect patients to community and social support services.
- Facilitate interaction and communication with health care staff and providers.
- Identify personnel in departments involved in the care of breast health patients (i.e. physicians, nurses, radiology staff, social services staff, radiation oncology staff, hematology/oncology clinic staff, genetic counselor, nutritionist, etc.) and develop relationships with them; offer educational sessions to inform practitioners of breast health patient navigator role, services provided and encourage referrals.

- Track interventions and outcomes.
MINIMUM QUALIFICATIONS:
To qualify you must have a High School Diploma and minimum of 1 year related experience in an office setting or the equivalent combination of education and experience is required.Effective Oral, written communication and interpersonal skills.
PREFERRED QUALIFICATIONS:
2-5 years of experience with breast cancer patients.
