Job Title: Hospital/Clinical Pharmacist

Position Overview:
We are seeking a skilled and detail-oriented Hospital/Clinical Pharmacist to join our team in Morrisonville, NY. The ideal candidate will have at least 2 years of hospital pharmacy experience and familiarity with Epic systems. This role is crucial to ensuring safe, effective medication use, collaborating with healthcare professionals, and optimizing patient outcomes.
Key Responsibilities:
- Review and verify medication orders for accuracy and appropriateness.
- Collaborate with physicians, nurses, and healthcare teams to develop effective medication plans.
- Provide guidance on drug therapy, potential interactions, and side effects.
- Ensure accurate dispensing of medications while adhering to hospital policies and regulatory standards.
- Utilize Epic electronic health records (EHR) to manage and track patient prescriptions.
- Educate patients and hospital staff on proper medication use and safety protocols.
- Monitor patient progress and make adjustments to prescriptions as necessary.
Qualifications:
- Doctor of Pharmacy (Pharm.D.) degree.
- Minimum of 2 years of hospital/clinical pharmacy experience.
- Epic systems training or experience is highly desirable.
- Strong knowledge of drug therapies, interactions, and clinical guidelines.
- Excellent communication and collaboration skills.
