JOB SUMMARY:
The Manager of Care Management (MCM) is accountable for the operational management of an assigned area for assisting the Admin Director of Care Management with human resource, quality, and financial management of assigned area. The MCM oversees implementation of care management functions in assigned area. The MCM serves as a clinical leader and resource in care management. This manager assists the Admin director with the implementation of plans and policies, and adherence to standards. The MCM assists with evaluation and reports relevant to achievement of goals. The MCM provides oversight to ensure area staff utilization patterns are congruent to standards. The MCM has the authority to make operational and human resource decisions relevant to his or her assigned area as long as decisions are consistent with Harris Health policies, standards, and other approved guidelines.
The MCM is responsible for facilitating an environment that supports professional practice and empowers staff to provide safe, effective, compassionate, and efficient care in collaboration with all health care disciplines. The MCM ensures compliance with standards established by professional organizations; national, state, and local regulatory agencies; and institutional policies and procedures. The MCM is accountable for practice that upholds high standards of integrity and ethical behavior. The MCM mentors others, role models professionalism, and participates in activities such as professional organizations.
MINIMUM QUALIFICATIONS:
Education/Licensure/Specialized Training:
- Education:
Graduation from an accredited School of Nursing with a Bachelor's degree in Nursing required. Master's degree in Nursing or related field is preferred.
OR
Graduation from an accredited school of Social Work with a Master's degree in Social Work. - Licensure/Certification:
Licensed to practice nursing in the State of Texas.
Related specialty certification required within two years of employment.
OR
Licensed Master Social Worker (LMSW) - minimum.
Licensed Clinical Social Worker (LCSW) - preferred.
Related specialty certification required within two years of employment.
Work Experience: Three (3) years of work experience in clinical case management.
Management/Experience: Two (2) years related management/leadership experience is required.
