Home Forward is seeking a limited duration Case Manager to support our Community Services department. The Case Manager position is responsible for providing housing stabilization services to Home Forward’s most vulnerable residents who are at risk of becoming homeless. Case Managers provide crisis response, risk assessment, and crisis stabilization planning for participants and residents residing in Home Forward housing. This work engages individuals with a focus on supporting trauma-informed care and racial justice.
Please note that this is a limited duration position; it is scheduled to conclude at the end of November 2025.This position is represented by AFSCME Local 3135 and is scheduled to close on Tuesday, October 29, 2024 at 11:59 p.m..The hours are Monday-Friday, 8:00 am - 4:30 pm.

- One (1) or more years of experience in behavioral health or chemical dependency case management AND a Bachelor’s degree with a focus in behavioral health or another related field is required.
