The Caswood Group is hiring 3 different LPN positions for a local LTC facility. We have 5x8 Days, 5x8 Evenings, 5x8 Nights and alternating weekends for each position. This is a 3 months contract position starting first week of March.
Responsibilities
- Accepts assignments from the Director/Manager of care areas to include, as indicated, delegating care assignments and/or changes to assignments and reviewing completion of assignments and related documentation at the end of each shift.
- Makes rounds to ensure that residents are receiving care outlined in their Interdisciplinary care plan and to address any concerns at the point of service.
- Assists in communications with other departments by placing calls or attending relevant meetings.
- Notifies or ensures notification of medical services of new orders, takes phone orders and transcribes and institutes medical orders on a timely basis.
- Ensures clinical update to staff via resident care records, interim care plans, shift‐to‐shift report, report sheet, clinical meetings, and staff meetings.
- Promotes positive and therapeutic environment for care and service for residents, patients, families, guests and personnel.
- Assists in identifying clinical education needs of staff and participates in education and orientation in the care areas.
- Participates in staff development regarding record of discussion and counseling to ensure adherence to Policies and Procedures and regulations.
- Follows up and assists in incident report investigations.
- Serve in the on‐call pool as appropriate.
- Monitors staff adherence to work expectations for promptness of work start times, break times, meal times, call offs and serves as a role model for these areas.
- Ensure that the crash cart, suction machines, and bladder scanner are kept in good repair, are checked daily, and are sanitized according to infection prevention procedures.
- Ensures that procedures for resident care are completed on a timely basis including, but not limited to, weights, showers, baths, meal board, patient communication boards etc.
General
- Adheres to dress code and maintains a professional appearance at all times.
- Performs other duties as assigned.
- Ensures compliance with regulatory criteria, policies and procedures of the facilities and safety.
- Respects the confidentiality of patients (HIPAA), departments and nursing home information.
- Displays employee identification badge, photo side visible at all times.
- Demonstrates a willingness to learn new procedures by asking questions and/or demonstration.
Care Responsibilities
- Performs as a Team Leader when the need arises and document relevant information on appropriate forms.
- May assist in treatments within the LPN scope of practice.
- Completes MDS’s assigned by the Director/Manager/Clinical Assessment Manager in accordance with regulation and accurately reflecting the health care status of residents.
- Attends care plan meetings and other relevant meetings as assigned.
- Participates in Quality Improvement activities including problem solving, audits, use of quality improvement tools and Focused Studies.
- Supervise and evaluate staff performance of daily assignments.
- Participate in admission and discharge planning processes as assigned.
- Promptly reports any suspected abuse, mistreatment, neglect, medication errors and any other unusual occurrences to the relevant director.
- Utilizes personal protective equipment, engineering and work practice control to prevent/minimize occupational exposure to blood and other potentially infectious materials and immediately reports any exposures.
Qualifications
- Active Unencumbered NYS PN License
- 2 years of LPN experience
- Preferred: 2 years long term care experience
