Your Role on the Team
Position Summary:
The Medication Management Specialist Pharmacist 5 is responsible for the provision of patient-centered clinical services, including review of patient medical records, performance of comprehensive medication therapy review, provision of medication information/education to providers and patients, application of evidenced-based medicine and national guidelines to optimize medication-related health outcomes. The Medication Management Specialist will provide care for patients in-person and virtually via video, telephone, or text as part of a pharmacist-led, referral-based, Medication Management Clinic. The role will ensure the appropriate selection, dosing, administration, and dispensing of medication orders and work under a collaborative practice agreement to autonomously manage referred patients. The Specialist Pharmacist will serve as a subject matter expert and complete comprehensive medication reviews for patients with specific high-risk medication concerns and high-risk disease states and patients who are at high risk for rehospitalization. The Specialist Pharmacist may perform prior authorization reviews of high-cost medications and deferred authorization requests, as needed. The Specialist Pharmacist is responsible for supervising pharmacy technicians, ensuring completion of medication histories, and actively precepting pharmacy residents, students, technicians, or interns as assigned by the supervisor. The Specialist will undertake tasks and projects to improve therapeutic and financial outcomes. Incumbents will lecture two times per year to other medical professionals and be published at least one time every three years in a medical journal. May be a co-principal investigator on patient clinical research, or an investigator on Pharmacy practice research. This position is a Direct Patient Care Provider as defined in Pharmacy Policy.

Required Qualifications:
- Registered Pharmacist Required
- Must demonstrate customer service and teamwork skills appropriate to the job Required
- Minimum four years of experience as a licensed pharmacist in a retail or community pharmacy setting, or two years' experience & completion of a PGY-1 Residency Required
- Knowledge and experience with the 340B drug pricing program Required
- Graduation from an accredited School of Pharmacy with a Doctor of Pharmacy degree Required
- Excellent planning, organization and time management skills Required
- Demonstrated experience providing oversight, training, and development for pharmacists and pharmacy technicians Required
- Demonstrated experience monitoring quality and efficiency utilizing metrics Required
- Demonstrated experience developing efficient workflows, programs, policies and standard operating procedures Required
- Demonstrate clear, concise verbal and written communication skills Required
- Current BLS or ACLS certification Required
- CA Board of Pharmacy Registered Pharmacist licensure in good standing Required
- Ability to establish and maintain effective working relationships across the Health System Required
Preferred Qualifications:
- Significant experience (more than 2 years) developing and implementing drug utilization programs for both inpatient and outpatient populations
- Effective presentation skills
