Case Manager/Youth Navigation
Methuen
Wayside Youth & Family Support Network has a Full-time opportunity for a compassionate and dedicated Case Manager acting as a Youth Navigator to provide case management and support for homeless and at-risk young adults (18-25) in the community. The Youth Navigator provides advocacy, support, and community connection to assist young adults to overcome barriers to self-sufficiency and stability. This can include though not limited to assessment of individuals abilities regarding transportation, medication administration, health/wellness, budgeting and substance abuse and development of treatment plans around relevant needs.
Salary Range: 23.00/hour + Sign on Bonus (half paid after 30 days of employment and half paid after 90 days of employment/pro-rated for part-time).
What You Will Do:
- Supporting young adults in accessing basic needs.
- Assessing young adults’ stability through use of the principles of Positive Youth Development, Trauma Framework ARC Foundations, Permanency, and Motivational Interviewing models.
- Hold regularly scheduled meetings with the young adults in their home communities of choice.
- Networking and building relationships with community partners and resources as well as internally within Wayside.
- Management of young adult paperwork including completion of yearly assessments, meeting progress notes and service plans.
- Participation in area System of Care meetings as well as regular collaboration with the projects Youth Action Board.
- Managing multiple priorities independently in a fast-paced environment.

- Demonstrates good therapeutic skills.
- Demonstrates ability to handle client crisis with maturity and good judgement.
- Demonstrates good assessment and treatment planning skills.
- Demonstrates ability to work with others (i.e. strong teamwork, can collaborate and coordinate care with other staff).
- Demonstrates high quality writing skills as evidenced by good client documentation.
- Demonstrates leadership skills (i.e. redirecting appropriately, maintaining a strong presence, being vocal in staff meetings, etc.).
- Bachelor's degree preferred.
- 1 to 2 years of prior experience working with young adults at risk to access community resources to assist in overcoming barriers of housing instability preferred.
- Valid driver's license is required.
- Bilingual/Bicultural candidates are strongly encouraged to apply.
