Title: Service Coordinator (Mental Health Case Manager)
Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.
Schedule:
- Full-Time: AM (Friday-Tuesday)
Qualifications:
- Prefer a bachelor’s degree in psychology, social work, or a field related to mental health.
- Two (2) years of full-time experience or part-time equivalent in a mental health setting.
- Demonstrated knowledge of DSM-V and psychosocial rehabilitation and recovery principles.

About the Campus: Crestwood Solano’s 16-bed Psychiatric Health Facility (PHF) provides an alternative to traditional psychiatric care through collaboration, empowerment, a healing environment, peer providers, family partners, and a recovery-oriented milieu. The PHF provides the structure and support to promote stabilization and foster recovery. This program serves clients experiencing a psychiatric crisis of such magnitude that it is unmanageable in the community. Our goals are to restore a sense of hope, self-empowerment, and realized potential of recovery in each of our clients and to provide a transition plan to enable a successful return to their community.
