Summary:
The Care Coordination Manager provides support and supervision to assigned social service programs. Additionally, this position holds responsibility for administrative functions such as staff recruitment and evaluation, reporting, and monitoring program effectiveness.
Essential Duties and Responsibilities:
- Provides weekly or bi-weekly supervision to program staff.
- Organizes workflow and ensures that employees understand their duties and delegated tasks.
- Coordinates staff scheduling and program coverage; provides direct service to clients during peak periods of service or during staff shortages.
- Reviews client records for accuracy and completeness in compliance with Contractor, State and Federal standards.
- Monitors contract compliance in relation to program implementation and best practices.
- Serves as liaison with program funders; develops and maintains professional working relationships with State and local healthcare organizations and providers.
- Collaborates with external partners to cultivate best outcomes for clients and families.
- Coordinates program activities with other program and admin departments and with external partners.
- Coordinates communication and information flow between program staff, funder/partner staff, internal departments, and external health and social service providers.
- Screen cases and referrals for intake and assign for assistance and follow-up.
- Offers guidance with complex client situations; provides supportive counseling and consultation.
- Assists director with planning and program development.
- Assists with billing, grant writing proposals, and other funding concerns as needed; monitors payment processes, reviews documentation and reports, and assists with problems/issues as needed.
- Provides input to the Program Director for the annual budget development process; ensures that budget targets are met.
- Assists in completion of program funding reports and assures all reports to funders and other entities as required are submitted in a timely manner.
- Collaborates with vendor/agency administrative and other staff on interpretation and implementation of new standards and policies.
- Assists director with developing, preparing and presenting reports.
- Participates in measuring clinical outcomes, data procurement and analysis activities.
- Establishes and maintains positive working relationships with other departments within the agency, community partners and funders, and state and local healthcare organizations and providers.
- Participates in management team, departmental planning, and program meetings as required, as well as local, state, and regional collaborative meetings pertaining to program services.
- May provide backup coverage for other leadership staff.
- Regular and routine attendance.

Minimum Qualifications:
- Bachelors Degree in Social Work, Psychology, Social Services, Human Services, or Behavioral Sciences, and/or licensure in Counseling, Chemical Dependency, Nursing, or Physicians Assistant.
- Two years of experience in a health care or social services setting.
- Supervisory experience that demonstrates progression and growth.
- Interviewing and counseling skills with an ability to work in a team-based environment.
- Ability to work independently with appropriate coordination among project staff, partners and customers.
- Full understanding of HIPAA and HITECH statutes, including contract compliance.
- Ability to complete the Tailored Caregiver Assessment and Referral (TCARE) Assessor Certification and other identified required trainings within 60 days of starting this role.
- Excellent customer service skills and ability to relate well with diverse populations and staffing levels and demonstrate respect, care and concern for clients and staff.
- Highly organized, able to work in a fast-paced environment and demonstrate prioritization skills/effective time management.
- Demonstrated critical thinking skills.

SUMMARY OF BENEFITS
Employees working 25 hours or more are eligible to enroll in medical, dental, vision, basic life, and critical illness insurance, with an increased cost share for employees scheduled for 25-29 hours/week. Employees working 20 hours or more per week receive 11 designated paid holidays plus 2 "floating holidays" per year, as well as paid vacation and sick time. Employees age 21 and older are able to enroll in our 401K plan upon hire and, after one year and at least 1000 hours worked, will be eligible for our matching program.
