Position Summary: Cone Health is seeking highly motivated clinical Pharmacist(s) to fill several openings in our service lines for critical care/emergency med and inpatient pharmacy positions. Successful candidates should have advanced knowledge and expertise to quickly assess clinical data and deliver direct patient care for the variety of diseases that are treated at Cone Health. These positions are accountable for the management of all aspects of medication-use processes, including but not necessarily limited to, the following:
- Interpret and verify the appropriateness of provider entered medication orders according to individualized patient needs.
- Manage drug regimens by approved protocols in consultation with the medical staff.
- Demonstrate ownership for the welfare of the patient by performing medication related patient education, reconciliation, and assistance with optimizing the patient’s drug regimen prior to discharge.
- Prepare and dispense medications efficiently following existing standards of practice and the organization’s policies and procedures.
- Conduct / provide interdisciplinary and pharmacy department medication related education for pharmacy team members across the health system.
- Participate in the training and development of staff, residents and students.
- Supervise pharmacy technicians in all pharmacy related activities.
Required or desired credentials or experience of applicants:
Required Degree(s): Pharm.D.
Experience Desired: PGY1 residency or equivalent experience, PGY2 desired for specific areas like emergency medicine, critical care, or cardiology.
Desired Certification(s): Board Certified in either Pharmacotherapy, Critical Care, Emergency Medicine, Cardiology or eligible to seek certification within a year.
