Community Care is hiring for a Residential RN Case Manager who is committed to teamwork and providing great care to become a part of our compassionate caregiving team. The Residential RN Case Manager provides nursing care and services for our residents. This is a part-time opportunity (4 days a week) on our day shift for our Sacred Heart location at 1555 S. Layton Blvd. Milwaukee, WI 53215.
- Work in collaboration with the Site Management Team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem-solving related to member appointments, transportation, and home care needs.
- Conducts phone and in-person assessments using decision tools and resources to identify immediate and potential health needs.
- Completes all required documentation and charting and assists in the development and maintenance of member records.
Residential RN Case Manager Job Requirements:
- Education: Associate's Degree in Nursing Required; Bachelor's Degree in Nursing is preferred.
- Experience: A minimum of 2 years of personal, clinical leadership, and/or employment experience providing care or service for a frail or elderly population required. 2 years of recent case management preferred. Experience in a long-term care setting preferred.
- Licensure: A current Wisconsin Registered Nurse license in good standing.
- Certification: Current CPR/AED certification.
Residential RN Case Manager Job Schedule:
- Monday - Friday; Work Hours are 8:00 am to 4:30 pm (32 hours per week).
- On-call rotation; weekends and holidays required.
