CD duties to include:
- Supervises and acts as a clinical resource to Therapy Partners Group staff, both clinical and administrative.
- Work with RDO, SMA, and PCCs to maintain evaluation load and schedule compliance to meet company standards and requirements.
- Generation of statistical reports, analysis, and data synthesis of provided statistics in dashboard, development, and implementation of plans based on data synthesis.
- Participate in monthly meetings with the COO to review production reports.
- Participates in ongoing weekly meetings with RDO, biweekly regional meetings conveying the clinic condition and needs.
- Perform biweekly staff meetings and weekly PCC meetings, generating meeting notes.
- Maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in a positive work relationship with customers, co-workers, and managers.
- Conduct patient chart audits to ensure that appropriate documentation guidelines are being followed and in compliance with Medicare law.
- Monitor compliance with Quality Assurance Program guidelines and updates and reviews as necessary. Address any quality and/or procedural issues with health care provider staff.
- Participate in annual performance reviews of all clinical personnel and provide feedback to HRM to assist in development and training in accordance with TPG values.
- Participate in Company in-services and staff meetings, as necessary. Responsible for scheduling of clinical in-services according to the needs of the Company’s staff to ensure compliance with appropriate delivery of care standards, and awareness of advances and developments in treatment protocols in each discipline offered by Therapy Partners Group.
- This position is also expected to see patients as required to meet the scheduling requirements of Therapy Partners Group.
Existing PT duties include:
- Provide patient treatment according to referral and visit volume demands.
- Monitor all patient discharges in alignment with Company standards of care.
- Ensure every patient receives a thorough, comprehensive, and customized physical therapy regimen, while receiving a unique experience.
- Provide a written and hands-on evaluation and immediate treatment of incoming patient conditions to determine appropriate physical treatment programs.
- Ongoing scheduling of patients to ensure fluid treatment progression.
- Develop and implement Physical Therapy treatment programs based on each patient's particular needs.
- Assist patients with all phases of Physical Therapy treatment and techniques to include:
- exercise instruction (both in-clinic and take-home instruction)
- Ensure daily treatment notes for all patients are complete prior to end of day.
- Provide continuous communication with referring physicians and Physical Therapy Assistants regarding re-evaluation findings to continually modify and update treatment programs.
- Operate and apply knowledge of standard physical therapy equipment.
- Ensure billing amounts on daily notes match treatment time and that all units of treatment are accounted for.
- Ensure billing is accurate and complete prior to signing off on the daily note in accordance with Company policy, state practice act, and federal regulations, including but not limited to HIPAA, Medicare and Medicaid.
- Ensure all patients are discharged with a positive outcome and experience or have been proactively referred to the next step to address limitations and/or complaints originally presented.

Education and Experience
- Experience: Minimum two-years clinical experience as a physical therapist.
- Education: Bachelor’s degree in health-related field and a Graduate Degree and/or Doctoral Degree in Physical Therapy.
