Caring Hearts Homecare is looking to hire an Intake Nurse to assist in the intake process and day-to-day supervision of direct patient care.
Qualifications:
- Must have a current RN/LPN license in Massachusetts.
- Background in public health, home health, or hospital work.
- At least one year of clinical experience.
- Hands-on nursing experience.
- Home care experience preferred.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Good computer skills.
Responsibilities:
- Assist in taking referrals and assigning patients to field staff.
- Follow up on referrals by contacting patients, physicians, family members, etc.
- Assist in handling patient care supervision and day-to-day patient care management.
- Assist in managing the HR needs of the clinical department, notifying the Branch Manager/Administrator when new hires are needed to stay proactive with the growth plan.
- Assist with new hire screenings/interviews of home health care personnel.
- Orient professional staff to clinical policies and procedures of our Agency.
- Perform other tasks as assigned.
