Job Description
The ideal candidate will work within a multidisciplinary team to provide physicals and medical care to existing clients who received non-medical services and new clients as they join the practice. They will be expected to conduct initial Physical Assessments and plan ongoing care for children and adults.
They will be responsible for:
- Conducting ongoing and wellness examinations and pharmacological evaluations.
- Prescribing medications, teaching clients about side effects, and monitoring responses to prescribed medications.
- Providing services to clients based on current status, risk, and as clinically indicated.
- Supporting staff to provide recovery-oriented and trauma-informed services.
- Participating in staff meetings.
- Providing crisis intervention services, supporting staff as needed.
- Communicating with hospital ER attending when clients access emergency services.
- Supporting discharge planning with hospital and ER staff.
- Conducting risk assessments, providing guidance and direction on risk management.
- Using e-prescribing.
- Completing progress notes, significant events, referrals, and evaluations within expected timeframes.
- Providing individual counseling as part of the plan of care if and when needed.
- Using Electronic Medical Record (EMR) and data reports from EMR to track treatment progress and individual client outcomes.
- Using collaborative documentation procedures for assessment and individual sessions with clients.
- Communicating with specialists and the behavioral health team to deliver integrated treatment.
- Working in collaboration with the client and significant others to support recovery goals.
Qualifications:
- Valid Maryland and DC or Virginia license for practice as a Family Nurse Practitioner.
- Extensive experience working with those with mental health disorders, co-occurring substance use disorders, and primary health care problems.
- Excellent written and communication skills.
- To be eligible to obtain and maintain employment, you must have a license that is free and clear.
- Knowledgeable about holistic care approaches, lifestyle modification programs, and medically supervised weight loss programs is a plus.
Additional Information:
Great environment and culture; seeking an optimistic and open-minded individual to start immediately.
Works within a collaborative multispecialty setting.
Needed to start one day a week on Thursday and increase days as census increases.
Hours: 11:00 am - 8:00 pm (office closed for lunch between 1:00 pm - 2:00 pm).
In-office opportunity but will see some clients via Telemedicine if and when needed.
